Summary: The episode was written by Ross-Leming and Bruckner and directed by Jensen Ackles, who always does a superb job directing, as he did tonight.
The episode follows three threads.
Sam injects DemonDean with sanctified blood. The scenes between the brothers focus on Dean taunting Sam and we find out that Sam was Lester’s demon deal contact. The ritual seems to be killing Dean slowly and Sam is worried.
The other thread shows the aftermath for Crowley and his quasi-bromantic affair with Dean. He’s presiding over trials and a demon burns himself before residing any longer in Crowley’s hell.
The final thread is a weakened Cas trying to get to the bunker and is waylaid by Adina, who shows up to kill Hannah and Cas for killing Daniel and trying to make her go back to heaven.
The threads come together as Demon Dean escapes the trap because he had enough human juice inside him to break the trap. He stalks Sam ala The Shining while Crowley intervenes with Adina. Crowley gives Castiel the grace from the dead Adina and then charges him to go intervene with Dean, kill him if he has to. Castiel shows up just as Dean is about to kill Sam. Cut to the devil’s trap and one last injection. Demon Dean’s eyes are black and then fade to human and he jokingly says they look worried.
Final sequence shows Sam and Cas discussing Dean. Sam goes off to get food and Castiel speaks to Dean about resting and being with his brother and also reminding Dean that the mark is still an issue. Final scene shows a red head in a chair somewhere in a Tulsa, Oklahoma hotel room who has blood dripping down on her from two dead bodies on a ceiling.
