There have been many, many, injuries, especially if your name is Sam Winchester. Here's a few of the more memorable ones, in no particular order.
Knee to the Groin - "After School Special"
Just the phrase "Knee to the groin" alone should cause some cringing, especially for you guys!
Broken Leg/Suffocation to Death by Angels ("Sympathy For The Devil")
Zachariah was onto something. A broken leg won't deter Sam? Fine, let's see how he does without lungs!
Bleeding Out From A Sliced Open Artery - "Jump The Shark"

Yeah, this one kind of set off a chain of events that to this day are burning The Winchesters.

Come on, admit it. The glitter bomb was a total payoff, wasn't it?
Struck by lightning - "Wishful Thinking"
Whoever said death is a career move never met Sam Winchester. Too many do-overs I guess.
Major Facial Damage by Pissed Off Demon - "Devil's Trap"
I have no idea why Sam isn't a vegetable after beatings like this.
Fingernail Ripped Off - "A Very Supernatural Christmas"
This one still gives me this heebie geebies. Yikes!
Shot to Death - "Dark Side of The Moon"
Is it so wrong of me to say this is the prettiest bloody corpse you've ever seen?
Nutcracker! - "Changing Channels"
I think this visual says it all.
Skull Cracked By Tire Iron ("Hello Cruel World," "The Girl Next Door")
Considering this led to a grand mal seizure in the ambulance in the next scene, double bonus!
Beaten Bloody and Unconscious (then beaten some more after that) by Dean - "You Can't Handle The Truth"
Remind me never to get on Dean's wrong side.
Bullet Wound to the Torso - "Mystery Spot"
Stitching up your own wound is pretty badass! This can also be applied to Sam stitching up the gash in his arm in "I Know What You Did Last Summer."
Sets Himself On Fire and Then Knocks Himself Out - "Bad Day at Black Rock"
To think this isn't even the worst of it. After this he's duct taped to a chair and beaten.
Impaled By Pipe, Suffers Premature Death - "The Song Remains The Same"
That's gotta hurt.
A Drive By Nazi Necromancer Darting - "Everybody Hates Hitler"
Yes, this is as bizarre as it sounds. Sam wasn't at all well though.

Maybe you could add every time Sam was suffocated? This article would probably be as long as a novel if you did though?
Does illness count? If so you could pretty much add in the second half of season 8. For us Sam!hurt junkies, season 8 was an unexpected boon in that respect!
Are we avoiding thinking about the eyescoop thing? The one that was already scooping Sam's eye when Dean shot his attacker?

So, when defining "injuries," I had to make sure I didn't expand that to curses, illnesses, mental issues, hallucinations, etc. I didn't even include the broken arm in season two, because Jared broke his wrist for real. I also had to eliminate "near misses," aka the mellon baller to the eye. Just plain physical injuries, and the more agonizing and cringeworthy the better!
So, I hope that explains the choices. Great suggestions though!

Another arm sliced. Sam's blood from both arms gushes into bowls under the table. Sam can do nothing but struggle.
Dean arrives and kills the ghouls, but is it too late? Sam's vacant stare and lack of movement on the table hints it is.
No, big brother makes it all better. I'm still not certain how given the deep gashes and blood loss (a hospital should have been involved), but he does.
Then there's the 'should really have been scarred for life' cuts to the face from 'Shadow'. I am pretty sure this must have been the episode that resulted in the invention of what the J's now refer to as 'network cream' - for the healing of scars in less than a week

This will of course be a great example of gruesome Dean injuries too.
Also how far do you reckon the camera is away from JP ? I assume there is really good zoom or it must be difficult not to go cross-eyed occasionally.
